What Causes Presence Of Crystals In Urine?

Posted on Thu, 31 Jul 2014
Question: since 2 weeks, I noticed one day that my urine was darker than usual. I had thought it was something I ate and feared that it was blood. so i made an appointment the next day with my doctor. he figured it was probably an infection or kidney stones but wasn't sure until the lab results came back. He did notices little traces of blood when testing my urine but again, he had to wait to see my urine results. in the meantime, he prescribed antibiotics in case it was an infection. a few days later, they results came back negative no infection. Since it was July 4th weekend, his office was close and had asked to see me to test my urine and do an ultrasound on my kidneys. I have no pains...the only thing i feel in my lower back, is a slight discomfort something that you get when you do pull-ups or bends which I have mentioned to the doctor.During this whole time, my urine has looked normal and sometimes it looks darker. Still fearing it could be blood again, I urinated in a bottle, it looked fine but I cant tell... I tried it again at different time and noticed it was darker yellow. but couldn't tell. so I continued with drinking plenty of water..my urine looked fine in sometimes it would look dark sometimes during the day but I couldn't when it would happen since it was random. My wife will kill me and it sounds rather disgusting, but I was curious to see if i would see something if i were closer... so i urinated in the sink and noticed visible brownish particles in my urine. I noticed this two occasions when i urinated and had drained the urine from the sink. I took a napkin and wiped the sink and confirmed it was this brownish particle. it even look like crystal shine to it.. I took to the internet and searched and in one case, I saw someone posted a similar issue and the doctor had replied to have themselves checked out ...could be kidney stones or one thing I did notice they mentioned was too many vitamins?? In addition to the multivitamins and medication, I do take fiber and started to take vitamin b-12 1000mg and D3 once a day.. My doctor asked me to take the Vitamin D because it was low in my last routine visit 3 months ago.. could this be the cause? I havent taken this morning and will continue with the original vitamins as I was taking since I was first diagnosed 4 years ago. those two vitamin D and B12 have been the only ones I recently taken in the past 3 months. Im just a bit worried since this never happened before and I hope its not life threatening.

Brief Answer:
Crystalluria

Detailed Answer:
Hi,

I have gone through your query. As you have mentioned that you felt crystals from urine, it is called crystalluria.
It might be due to kidney stones, ureter stone or infection in Urinary tract.
Your infection has been ruled out by lab test and in case of stones there may be severe pain in flanks radiating to inner thighs. Ultrasound will rule this out later when you will have it.

Vitamin D and B12 will not cause crystals in urine and if there is data on internet, 3 months of intake is a negligible amount.

Now coming to the point. Since you have Diabetes and you are on Glyburide (Glibenclamide) and metformin combination, Glyburide is considered to cause crystalluria. Other sulfonamides also cause same problem.

You have to continue your diabetic medicines, no need to change them. Wait for your Ultrasound report and consult with your treating doctor.
For your problem, drinking plenty of water will help you and later on you can consult your Diabetologist for other options.

Hope I have answered your query. If you have any further questions I will be happy to help. You can contact me on my page http://bit.ly/DrGaurav

Doctors,

Thank you for replying. I tried to be as detailed as possible. I have been drinking plenty of water and noticed a of color change to normal. I haven't seen remnants of the brown particles after urinating. I guess i seen it more this morning and as drank more water throughout the day, the color of the urine is back to its normal color (a light yellow). As for the Kidney stones, I don't feel or experienced any pain which is what my doctor had checked for. I did mentioned to him that I have a slight discomfort on my lower back but I don't know if its the exercise I started doing like situps..it been a while since ive done them and I get the discomfort sourness when doing certain exercises that requires your muscles that you haven't really used. I've read several articles on site and some wont give me too much information or point to Kidney Stones and can be painful.

Brief Answer:
Kidney stone needs to be ruled out

Detailed Answer:
Hi,

Thanks for the follow-up.
I understand about your concern towards Nephrolithiasis (Kidney stones). What you are getting over internet about kidney stones are the symptoms of a later stage of the condition, when it becomes symptomatic. At initial stages, when stone is smaller in size, it doesn't show any symptoms of pain. It is diagnosed accidentally when someone do a ultrasound.

This is why your treating doctor wants to confirm that your problem is not due to stones, by doing ultrasound. If the result came out to be negative, then other causes can be considered like i said about side effect of Glyburide.

Color change of urine is because of fluid intake causing dilution of urine. Excessive drinking of water is advised because it will force out any crystals remaining and decrease the chance of infection which may occur if crystals erode inner lining of urinary tract.

Condition is not life threatening. But you have to follow your doctors advise to rule out the causes.

Hope I have answered your query. If you have any further questions I will be happy to help.If you do not have any clarifications, you can close the discussion and rate the answer. Wish you good health.

Thank you again for following up. I've been taking my medication for diabetes since I was diagnosed 4 years ago. I haven't had any problems and I try to eat right..I test my sugar regularly and I try to keep my sugar levels as close the mid 90s but it varies and have seen it go over 120...but that's because I cheat a bit but i quickly return to my new eating habits..it's a wakeup call and monitoring helps me to maintain my sugar levels at good rates.

Brief Answer:
good control

Detailed Answer:
Hi,

I am glad to know that you are maintaining sugar level at normal range. Slight fluctuation happens most of the time. To see the actual control of sugar level, glycated Hb (HbA1c) is needed. Being a diabetic you should get it done after every 3 to 4 months.

To your present case, i hope drinking water is helping you out. Sugar level has nothing to do with crystals in urine.
I will be happy to follow you up once your investigation reports are available.
