Brief Answer:
Exercise induced bronchitis

Detailed Answer:
Hi
Thanks for writing to us.
From your description it looks like your grandson has exercise induced bronchitis or asthma.
It is commonly seen in children and is due to shrinking of airpassages with exercise or play.
I usually recommend Complete physical examination and chest xray to rule out infections.
If infections are ruled out then I prescribe salbutamol inhalers one hour before he starts playing or running.
I also add monteleukast at 4 mg daily in the evening to decrease the allergic reaction.
Wishing your grandson speedy recovery.
Regards
Dr Arun
