What Causes Purple Discoloration On Sole Of Foot Along With Toe Pain?

default
Posted on Sat, 28 Mar 2015
Question: 3 months ago, I had severe bleeding into the skin forming a purple sole that covered front half of sole of right foot. Toes also turned purple and then black and were amputated to mostly eliminate severe toe pain.. Rest of foot shows no improvement and in fact is in much pain on contact halfway along right side and at the bunion, and the ball. Bleeding seems to be spreading a little bit further on the left side. My doctors here and at XXXXXXX don't have a certain diagnosis nor what I can try. Thanks--

Brief Answer:
You may have gangrene in your foot.

Detailed Answer:
Hi, dear
I have gone through your question.
I can understand your concern.
You may have gangrene in your foot. You may have venous gangrene due to some obstruction in the venous flow. Less likely you may have gangrene due to low blood supply or diabetes.

Please give me some history.
1. Do you have diabetes?
2. Do you have any trauma or injury?
3. Do you have hypertension?
4. Are you a chronic smoker?
5. Do you have other illness?

if possible send me your reports and photograph of that area.

I will wait for your reply.

No diabetes, no trauma, no smoking in last 35 years. I have hypertension which seems well controlled right now with Cardizem and Metorpolol at ~145/90 although sometimes spikes to over 170/110. Have had Afib diagnosed in 2008 and was on lisinopril and metorpolol. Switched from lisinopril to cardizem at onset of current problems 3 months ago. In 2010, DVT's and pulmonary embolism. In 2012, amputation of necrotic little toe on right foot. Blood tests later revealed antiphospholipid syndrome.

Current problems started about 11/1/2014, per my earlier description. I had had a root canal in October, followed by a week of amoxicillin. About 11/1, the foot became so painful, I couldn't stand on it. Several weeks in hospital yielded no real improvement. Cardiogram(?) showed no blockages Foot pulses are excellent.Recently remaining toes (necrotic) have been amputated. They seem to be healing OK. But no real change in the purpling on the sole or in my ability to stand without severe pain. Rubor is severe whenever I hang my leg down.

Am unable to send pictures but will try further.

Brief Answer:
You may have vascular insufficiency.

Detailed Answer:
Hi, dear
I have gone through your question. I can understand your concern.
You have no trauma, no history of smoking so chance of peripheral vascular disease is less likely.
You have history of deep vein thrombosis and anti phospholipid syndrome so you have high chance of vascular insufficiency due to some intravascular clotting. I feel you will benefit with anticoauglation (warfarrin) therapy. Discuss with your doctor.

And yes, I await for the pictures so that I can reach to a diagnosis and help you further.

Thank you. I have been on warfarin therapy following AFib finding about 5 years ago. I'm still trying to get photos to you. I guess the question now is, will mother nature take it from here ? I have rather steady pain ( maybe 2-3 rating) which gets worse when I lower my leg and blood seems to rush in. I may be seeing slight improvement in pain day-to-day.

I am sleeping well through almost every night.

Brief Answer:
It is improving.

Detailed Answer:
Hi, dear
I have gone through your question. I can understand your concern. You should continue your warferrin. Your pain is improving. So no need to worry. Just wait and watch approach should be followed. Go for regular examination. If it get worsen then chances of gangrene is there so keep close watch on it. And try to send photograph.
Hope I have answered your question, if you have doubt then I will be happy to answer. Thanks for using health care magic. Wish you a very good health.
