I have read your query. I have seen the attached photographs also. I understand your concern.
Adaplene is a very good medication for acne but it may cause irritant contact dermatitis in few people. Following are the main reasons often implicated in causing adverse reactions-
1) if applied in excess amount
2) if rubbed vigorously
3) combined with other peeling formulations
4) person has excessively sensitive skin
5) if applied on already inflammed skin
I always advice my patients to apply adaplene in a very small quantity very precisely over the acne only. It should not be applied all over the face.
First time user should better wash it in 20 minutes and slowly increase the contact time everyday . This makes the skin tolerant to adaplene.
Since now she has developed irritant reaction, better she should stop using it now.
She should apply liberal amount of moisturiser over the face for few days.
Stop all other topical medications .
Use gentle facewash .
Take antihistaminics like levocetrizine.
It might take 1-2 weeks before the skin gets normal.
