What is the cause of it?
Based on facts of your query, you seem to suffer from Balanitis?
Which could be from drug induced contact dermatitis from topical ointment.

What to do?
Consult Dermatologist / Urologist / Surgeon,who would physically examine and would assess the cause of Balanitis? Viral or drug induced Dermatitis in your case.
Painful erections from balanitis.

Remedy-
Just check up with your doctor and you should change the ointment to steroid version for few days to control rash, if needed.
Tab NSAIDs-like ibuprofen / Tab Allegra-for allergic contact dermatitis-if need be.would resolve your issue soon.

Please update more details-
as whether rash on penile head occurred after ointment application?
Or whether the rash was first there and ointment was applied , after which it got flared up balanitis?
What is the name of the ointment?
Give more details about your complaints,its duration, about any diabetes?

Mild diabetes.

Hi,Thanks for updates.
Stop Vagisil-as you have allergy for it.
Painful erections are from glans irritation from allergic contact dermatitis from Vagisil.
Continue Cap Allegra/ pain killer ibuprofen,as suggested.

Please update on duration/ colour/size/location/ number of rashes on penile head.
