What Causes Recurrent Depressive Disorder?

Question: I've been suffering from mental illness since 2008 and was intially diagnosed with depression that same year, and in mid 2010 i was diagnosed with depression and psychosis, and put on 20mg of Fluoxtine in the morning, and 25mg of Quetiapine in the morning and 100mg at night, which has been very effective in making me very well, and being able to go back into full- time work and sport in late 2010, and in sport- perticulary table tennis and tennis being able to return to a very simular standard as before- playing number 1 and 2 in table tennis and number 1 in tennis in my respective teams, the first time since 2008 of being able to do very little. But since November last year i started to feel quiet unwell, which has progressed to me now being picticulary feeling very tired out and sleeping 15-16 hours a day, and feeling very hungry and thirsty most of the time. I've recently seen a new doctor and he took me off the 25mg of Quetiapine in the morning, and put me on 250 mg of lithium, and later the lithium was upped to 750 mg, and taken off 20mg of fluoxtine, and put on 50mg of Pristiq. Just before i was put on lithium, before i seen another doctor, i was feeling very unwell, and did some research, and upped the quetiapine out of desperation of how i was feeling by 25mg for 2 nights, and then upped it to 50mg and resumed taking 25mg of quetiapine in the morning, for a period of about a week, and found it to give signifient relief, and discontinued taking the extra quetiapine for about a week, and then then continuned taking the extra quetiapine until now for about the last 2 and a bit weeks. I find myself being confinded to the bed or couch for most of the time most days. I've continued to play sport, but have had to stop my main job shearing sheep, something i have done since late 2010 to November last year, and have been able to help my Dad a small amount of the time on the farms.

Brief Answer: Better to take antidepressant with quetiapine Detailed Answer: Hi Thanks for using healthcare magic. I, Dr Abhishek Kapoor, would help you in this query. I think, you are a case of recurrent depressive disorder (RDD), if there is no history of manic episode. In case of RDD, we can improve with antidepressant like fluoxetine or paroxetine. You can take quetiapine 25mg in morning and night. At low dose, quetiapine acts like antidepressant, so there is no harm in taking it. At night, you can take even higher dose of quetiapine up to 50 mg or more to get sound sleep. I would prefer to put you on antidepressant like fluoxetine or paroxetine with quetiapine rather than lithium with quetiapine. In case, you do not improve with antidepressant, then i would prefer to add lithium. Rest, you can discuss with your psychiatrist. In case, you need further help, you can ask. Brief Answer: Consult a psychiatrist Detailed Answer: Hi Yes, general physician are not specialist in mental disorders. They know only little bit about the disorders and with that knowledge, they try to treat patients. A psychiatrist would not put you on lithium for recurrent depressive disorder so early. According to guidelines, if a patient does not get benefit with antidepressants, then lithium can be tried, other wise keep him on antidepressants. Hi i seen my GP today, and he took me off lithium and put me on Valproate 500mg to be taken twice daily, and reduced me to only taking 50 mg at night, and thinks i have bipolar due to pressured speech. I rang a mental health worker later on about seeing a psychiatrist, and said i needed to get a referal from a GP, and it could be a number of weeks untill i can see a psychiatrist by video conference, which is obviously very concerning. Any thoughts??.

Brief Answer: Try to consult another GP or psychiatrist Detailed Answer: Hi Valproate is also a mood stabilizer like lithium and has lot of side effects. Only on basis of pressured speech, we can not say, patient has bipolar disorder. There should be other symptoms also like over-talkativeness, decrease need for sleep, irritability or euphoric mood, over spending or over grooming, increase sexual desire etc. If you do not have all such symptoms at any time of your life or any episode, then, there is no manic episode. In that case, your diagnosis would remain recurrent depressive disorder. I would suggest you to consult another GP or psychiatrist before taking valproate. Diagnosis depends upon observation and observation of a psychiatrist is better than general physician about mental disorders. In case, you need further help, you can ask. My GP has reduced me to taking only 50 mg of quetiapine at night, should i do this?, and what do you think of me taking desvenlafaxine , which took the place of fluoxtine a couple of weeks ago?. And should i keep taking the lithium?.

Brief Answer: You can take des-venlafaxine Detailed Answer: Hi You can take 50mg quetiapine at night. At this dose, you would get sound sleep. You can take des-venlafaxine, it is a effective antidepressant and does not have any major side effect. If you do not have any manic episode or symptoms, then there is no need to take lithium. It has lot of side effect like kidney dysfunction, acne, liver dysfunction, thyroid dysfunction etc. Better to avoid it, but before leaving it, consult a psychiatrist and diagnose yourself properly. My GP has taken me off lithium, and so don't really know what a safe dose would be if i keep taking it?. And could be a number of weeks untill i see a psychiatrist, even via telemed(video conference). What should i do?.

Brief Answer: You can take same dose of medicine Detailed Answer: Hi As i already told you, if you do not have any manic episode or symptoms, in that case, there is no need to take any mood stabilizer. Effective most of lithium is 900 mg. You were on 750 mg, that means, you were on below therapeutic dose of lithium. I would not suggest you to leave any drug without psychiatrist consultation or proper diagnosis. Currently, whatever medicines GP has prescribed you, you can take and try to get psychiatrist appointment as early as possible. Valproate has lesser side effects than lithium.
