What Causes Recurrent Fever Even After Taking T-98 DS?

Question: My son (9yrs) is having high fever 102-104 since 11 days. Blood reports are:-
Widal Test serum
Salmonella typhi O (TO) Reactive 1:40
Salmonella typhi H (TH) Reactive 1: 40
Salamonella paratyphi A, H (AH) Reactive 1:160
Salamonella paratyphi B, H (BH) Non Reactive

Blood Culture Report
No Aerobic pyogenic organism grown after 24 hrs
Organism : Salmonella typhi- isolated

We are giving him T-98 DS for fever but the fever is coming back after every 3-4 hrs.
Can u suggest me why it is happening.
Doctor has started giving antibiotics for typhoid.

It is Paratyphoid-A - I suggest Intravenous antibiotics

Detailed Answer:
Hi....I am glad that your doctor has done a blood culture before starting antibiotics.

This is now a confirmed case of Paratyphoid A. To be explaining in common terms - this is a cousin of Typhoid.

Usually rather than Typhoid - this Paratyphoid A will take longer time for the fever to settle down. Sometimes it can take as long as 18-21 days also for the fever to settle down. But you need not worry as now we have a diagnosis for the fever.

I suggest -

1. Intravenous antibiotics (Ceftriaxone at 100mg/kg/day) in 2 divided doses till the kid is afebrile for 24 hours and then we can change over to oral antibiotics.

For this you need to get admitted in a hospital which can take care of kids.

Is it necessary to intravenous antibiotics. As the doctor has asked us to give antibiotics for 5 days and wait
Ceftriaxone - shows sensitive in culture report, can it be given or some other medicine??

Hi... I am suggesting intravenous antibiotics as the fever is already there for almost 11 days.

If intravenous antibiotics are given the response to the drug will be fast and as soon as the fever settles - you can get discharged and shift over to oral medications which can be given at home itself.

This is the reason why I am suggesting intravenous antibiotics.
