What Causes Recurrent Nose Bleeds, Constant Abdominal Pain And Irregular Menstruation?

Question: My 19yo daughter suffers from recurrent nose bleeds, insomnia, constant abdominal pain, lack of hair growth (has had 1 haircut in a year) and irregular menses. She's beginning her sophomore year in college. She is taking Adderall and birth control. Wondering if you can put these symptoms together to guide our testing path? I thought of possibly hypothyroidism or polycystic ovarian syndrome. She hasn't had blood work in awhile, but nothing is coming up for her primary care physician. Thanks for any help!

Most of your daughter symptoms like insomnia, nose bleeds, abdominal pain and lack of hair growth can be side effects of Adderall.

On the other side, I can't find a valuable explanation why your daughter has irregular menses under hormonal pills. The birth control pills in young women are frequently prescribed to resolve the problem of irregular menses.

However, you're right. The hypothyroidism can explain most of her actual symptoms. If your daughter were my patient, I will first suggest doing the thyroid function tests(TSH,fT3, and fT4) to understand her actual thyroid function and confirm or exclude the diagnosis of hypothyroidism.

About the diagnosis of the Polycystic ovarian syndrome I can say that this is most probably because apart irregular periods, your daughter should have even other important symptoms like excess hair (hirsutism), hair loss, acne, weight gain, difficulties with fertility, increased anxiety and depression.For this reason, in my opinion, this diagnosis can be excluded.
