What Causes Recurring Anal Fissures?
Question: I am a 25 year old male who has suffered from chronic anal fissures. I had a lateral internal sphincterotomy 1.5 years ago but I'm still experiencing anal fissures. The fissures appear in the same location and it appears there is now scar tissue and it never fully heals. I have used Diltiazem and Nifedpine ointments for 2 years. What do I do next?
Brief Answer:
Needs assessment and conservative therapy, if fails then resurgery
Detailed Answer:
Thank you for asking
I read your question and i understand your concern. Anal fissure is very hard to get rid of some times and recurs with anal tags and fissures. You need to continue the topical ointments, add sitz bath to it, good bowel habits and all efforts to avoid constipation and not delaying bowel movements as that worsens fissure. If conservative measures fail, i am afraid you will have to undergo another surgical intervention to resolve these fissures. Let your surgeon decide what is best for you.
I hope it helps. Take good care of yourself and dont forget to close the discussion please.

Thank you. I have two follow-up questions.
- Does re-occurring fissure cause scar tissue that is not as strong as healthy tissue?
- I have attached two photos. Does this look like scar tissue?

Brief Answer:
Supportive care
Detailed Answer:

Anal fissure leads to anal tag. With good diet and lifestyle adjustments, good bowel habits , anal fissures resolve. Your pictures indicate a small anal tag with a fissure causing this pain. I believe conservative measures as mentioned above with topical vasodilators will help. If the issue persist anal dilator via surgical intervention will help. Talk to your surgeon and let them help you in this regard.
I hope it helps.
