Brief Answer:
Need Biopsy.Upload image if possible.
Detailed Answer:
Hi Sir,
I understand your concern. If possible please upload a photo of the lesions so that I am better able to help.
Going by your clinical description and age, its very important to get a biopsy of the lesion of the penile head, since it has been there for sometime.
In all such cases, one need to rule out malignancy of the penis or any pre-malignant lesions.
Further management depends exclusively on the test reports.
Post your further queries if any.

Detailed Answer:
Hi Sir,
I understand your concern.
I have gone through uploaded images as well.
The indentation has been prominent.
Considering your age and the location of the lesion ,I advise you to get a penile biopsy to rule out any neoplastic lesion.
Once any malignancy is ruled out, further management can be based on the test results.
