What Causes Reddish Spot On Penis Along With Redness Around Urethra?

Question: Hello ,

I have developed a red spot on my penis and redness around the urethra. I have been to the doctors and tested gonnerea chlamydia and herpes. The test came back positive for chlamydia and I was prescribed 4 tabs Azithromycin which I took the same day. It has now been 9 days and the redness has not subsided. Is this normal? Also, I thought if I may have a possible different std as well such as syphilis. Would the lab be able to see this std even though not specifically testing for it? The doctor also prescribed me sulfamethox for 10 days which I'm currently taking because I had some white blood cells in my urine. Said it could be an upcoming infection. Please help

I have gone through your query and I have noted down your complaints.

Chlamydia usually does not cause red spots on penis. Though there may be periurethral edema and some redness (due to urethritis)
Chlamydia is a cause of Non-Gonococcal Urtheritis (NGU) in males and patients usually present with burning sensation on passing urine and Clear/Transparent urethral discharge.
You were treated for chlamydia because you tested positive for chlamydia.
Syphilis, can have varied presentations Or it may be clinically silent e.g early syphilis may present either as a chancre(sore) Or it may present as mucous patches. Late syphilis is usually silent.
However, it is hard to explain why were you not screened for Syphilis and for that matter other STD's like HIV.

I would like you to upload a picture of the area of concern. The red spot and redness could be a simple candidal balanitis.

I would like to know in more details about your sexual history.

-Are you sexually active?
-Do you have single or multiple sexual partners? When was your last sexual contact? Was it condom protected?
-Do you have any urethral discharge or burning on passing urine? Or any other genital symptoms?

Yes I am sexual active. I have done nothing sexually since my concern and diagnosis. I am waiting For This redness to go away. Last time I have sexual contact was around the 20th of December 2014. I believe I do know how I contracted whatever this is through oral sex. My partner tooth was grazing the same area where the red spot developed. It initially developed raised and red with dry skin. The skin came off while showering that left a red spot. Also starting out there was 2 spots very close together. Since I have been on medication the red spots have sort of came together and flattened with no more skin flaking. Yes I do use condoms but have had unprotected a few times. When I urinate there is a slight if non burn.

I have reviewed the attachments. I would keep a likely possibility of Xerosis/Dryness of skin
I would suggest you to continue with the moisturizing cream preferably mixed with a topical steroid e.g either mometsone furoate Or betamethasone valerate, twice daily. You can check with you local pharmacy whether these steroids are available OTC in your region.
You may stop mixing the steroid after 2 weeks and thereafter continue with just moisturizing cream.
An OTC oral antihistamine e.g cetrizine 10 mg once daily will provide you symptomatic relief from itching.
You may change soap to either Dove or Pears.

Hello,

Dr. I have applied no creams that you have listed yet but I will look further into these Otc ointments. On your second statement about an itch? It has no itchiness at all. Although if I take my finger and rub across this area it seems to have no sensitivity as the non irritated spot. Please help
Do you still think it could be candidal balanitis like your original thought? Or even a bacteria? I have since spoken with you yesterday to be retested for all std and hiv. Will have results back sometime this week.

Hi.

Thank you for following it up. I want to explain it further to you.
The patches of dry skin (Xerosis) that I can see in this picture don't look like an STD.
Although the timing of appearance of these patches does lead one to suspect an STD but that is just a coincidence.

In the picture I can see well defined, dry patches with slightly scaly edges. I used a general term i.e Xerosis, which is term used for dry skin to make it easier for you to understand.
That is the reason I asked to continue with a moisturizer.
Actually this looks more like an eczema, probably psoriatic, which can present with dry (xerotic) scaly, red patches.
These patches of dry skin (likely to be psoriatic patches) would improve with a topical steroid for 2 weeks. Psoriasis is a steroids responsive condition and therefore I suggested you to apply a a steroid.
