Question: I have had a skin irritation in 2 places on my face. It itches sometimes, it's red and it flakes by morning. I've been using lotrimin for about 3-4 weeks now but it's not going away. Do you know what this is? I've never had this before

Brief Answer:
The important thing is to diagnose first.

Detailed Answer:
Hi XXXXXXX,

Welcome to health care magic,

This red, irritating and sometimes itchy skin places of face could be due to eczema or allergic hives or atopic dermatitis or tinea corporis etc.

Can you just take a clinical photograph of the area over face and upload,so that i have a better view in order to diagnose?

More than any other field, in dermatology one needs to inspect with eyes what the skin condition is,how it looks, how is that redness or itching etc

The important thing is to diagnose first rather than treatment.

You can go to skin specialist for detailed history and inspection skin of face to have a clear diagnosis.

Treatment is mainly depend upon the underlying cause.

You might require topical corticosteroid like beclomethasone, clobetasol or mometasome with antifungal cream like miconazole or itraconazole with antihistamines orally to reduce itching.

Some time oral antifungal once dosage with prednisolone are also be effective.

Do ask me a follow-up question.

Thanks and Regards,

Brief Answer:
Could be cellulitis or eczema or fungal infection.

Detailed Answer:
Hi XXXXX,

Thanks for the follow-up query,

It seems to be more of a furuncle, though I would still consider Eczema. It probably is not a fungal infection, but I just want to know if you have only that lesion, or is it there anywhere else on your body?

Antihistamines cream or ointment alone will not cure your problem though they may make it itch a lot less, but I think you should apply an antibiotic cream called Mupirocin (bactroban) on that area. It is available on prescription.

Better to try it for 3 days, daily twice, and do get back to me about the improvement. Depending on the condition then, I can suggest further management.

Thanks and Regards,

Dr. Sandip Kabra
