Dear Sir,
Rupture of Hemorrhoids is very rare, it is usually associated with torrential bleeding. Was it Hemorrhoid or peri anal abscess? Was there pus discharge after the rupture?
Redness of scrotal skin associated with painful buttock area could be due to associated fungal infection. If the bleeding in the stools is associated with pain(pain while passing stools) possibility of anal fissure needs to be ruled.
Possibility of following nees to be considered and ruled out before carrying out colonoscopy
1. Anal fissure: Its a small ulcer at the anal opening which usually occurs due to tear of junction of skin and anal opening usually due to passage of hard stools
2. Fungal infection of the scrotum and buttock fold
3. Other painful condition of buttock like pilonidal sinus needs to be ruled out.
I would suggest you to send us clinical photograph for further assessment of redness of scrotum and any lesion in the buttock area.

When I first had the large external Hemorrhoid it continually bled so I went to Urgent care, they referred me to another specialist with the intention to remove the Hemorrhoid. One week later he first saw me by that time he decided to let nature do its thing and did not recommend surgery. My problems have got progressively worse since then, making my life miserable and very uncomfortable in all positions, even standing now. Blood in stool still happening.

I am sorry for state of your affairs.
I would suggest you following
1. Hot seitz bath: To a tub of lukewarm water add handful of common table salt . Sit in it immersing your bottom for 15-20 minutes. After that you can have shower. It will relieve the pain and congestion
2. Xylocaine Ointment: Application of Xylocaine which is local anaesthetic will relieve the muscle spasm and reduce pain. Apply it 3-4 times a day, before and after passing stools
3. Oral Laxatives: Lactulose 10 ml twice a day will help in softening stools and reduce bleeding
4. Analgesic like Ibuprofen will be helpful
5. Plenty of oral fluid
6. High fiber diet
I would also suggest you to send clinical picture of scrotum for assessment .
