The toes and the metarsophalangeal joints need to be examined. If there is marked swelling and tenderness then joint fluid needs to be aspirated and sent for examination to look for conditions like gout, inflammatory arthritis and rarely septic arthritis though it needs to be ruled out especially if a single joint is involved. X ray foot can be done to look for evidence of erosions. CBC ,ESR and thyroid function tests can also be done as part of the workup. Paracetamol can be taken safely for the initial discomfort if its bothersome. If the workup is clear then paracetamol alone can be taken symptomatically.
