Hi.
Thanks for your query and an elucidate history.
I can understand the plight you must have been going through for about 15 months.
Your mentioning age and other details helps me a lot, please let me know such details too along with any other symptoms which you may feel not related to the present problems you have noted here.
To recapitulate salient points (for my sake, not to miss any points): Age / right testicle - dull pain - shoots up into body - especially when lifting or climbing stairs -
Sometimes you feel a *ripping pain in the bottom ''left part of stomach'' (not the right) in conjunction with this pain - feels that the testicle has **never sat right - It's like there a small pocket of fluid underneath it which I can feel that it's resting on not allowing it to sit in the sack right, and causing trauma, and there is the dull ache right wear it sits. - this is in spite 3 ultra sounds, and *2 ct scans showing nothing - treated for Epididymitis - 5 different doctors all urologists - car accident - stopped Gym - now doing more lifting and manual labor -
** last week - pain returns - peeing constantly - UTI treatment - no relief -
** Now - penis retracting into body - not filling out its foreskin when flaccid - no sexual desire - wife is pregnant so not sterile - testicle pain has subsided -
now the penis shaft retracting, constant urination, and discomfort on the tip of my penis is unbearable ))

Read and re-read the history and tried to link up the symptoms you are going through.
My thoughts:
- It is possible that there is infection of the whole uro-genital (urinary + genital) tract and manifesting at different points at different times.
- Started the presentation as epididymitis and then orchitis , then routine UTI and now most probably Prostatitis
- Pain in the 'right' scrotum but the ripping pain in the bottom 'left' part of stomach > difficult to explain, bu the reason then would have been Prostatitis.
- Recent problem related to penis: shaft retracting into body - if there is no sexual desire, this is the normal appearance one get.
- Constant urination can be due to cystitis (inflammation of the urinary bladder, particularly the ''trigone'' part which is near the outlet of bladder and then covered with prostate; prostatism causing other problems too.
- Pain in the testis can be a referred from the urinary bladder, prostate even the ureter.
- Feeling that the testicle never sat right > this is possible as due to the disease of the testis and/or scrotum there can be a very small ''secondary'' hydrocele that you must be feeling.
- wife pregnant - you are sterile > do you mean that you are not having any sexual contact or do not ejaculate at all.
This will add on to the problem of prostatic congestion.
I would advise you the following including the ''basics'' of care to be taken in such complicated case.
- Scrotal support using lungot.
- Plenty of water orally
- Not standing for a long time.
- No strenuous exercises or weight lifting at all.
- Regular ejaculation to empty the prostate, seminal vesicles, epididymis, testis.

You need a thorough clinical evaluation, actual physical examination and investigations:
- Per-rectal digital examination to palpate for the prostate, seminal vesicles, Prostatic massage to get the fluid for testing. This may act even as a therapeutic measure (old system forgotten by the new-gen Doctors)
This gives the information that no other test can give. A trained finger tells the most.
- Palpation of the epididymis, testis and the spermatic cord.
- To check for inguinal Hernia.
*Investigations:
- Intravenous pyelography in a traditional way.
- per-rectal ultrasonography.
- MRI for Genito - urinary system.
- Any other tests that may be needed by the clinician on evaluation and examination.
A combined effort like this may help you to get a proper ''diagnosis''.
