Question: Hello there. The past few days I have been having right flank pain. I thought I just pulled a muscle. yesterday I was extremely nauseous and today I am throwing up. I am also very weak and tired. I noticed this weird rash up and down my arms, back, and upper legs. I also feel very cold. My legs feel very sore and still. My body also aches. Any idea what this could be?

Brief Answer:
Possible pyelonephritis (kidney infection)

Detailed Answer:
Hello XXXXXXX

I am sorry you are feeling so sick. Flank pain + nausea + possible fever (feeling very cold) are hallmarks of a possible kidney infection. I couldn't make out the rash very clearly, but likely the infection is causing the rash.

While some bacterial infections (sinusitis, bronchitis, ear infections) can wait and resolve without antibiotics, this kind of infection really does need an antibiotic. You should go in to either your doctor or an urgent care clinic or an ER. Make sure they do both a urinalysis and a "culture and sensitivity" on the urine. The latter test identifies what the bacteria is and tests it against which antibiotics are sensitive or resistant - in case whatever they prescribe you turns out not to work adequately. Given you (likely) fever, they should also get a complete blood count.

So do go in now. If possible, let me know later how you are doing.
