What Causes Right-sided Chest Pain When Diagnosed With Hiatal Hernia?

Question: I was diagnosed with a hiatal hernia several years ago. I have this annoying kinda pain more like a burning pain on the right side of my chest. It comes and goes but has been bothering me for about a week now. I have been taking Nexium but it is not helping. It seems to feel better if I eat or drink something cold or hot. It really does not bother me at night. Sometimes I can feel it in my back too. Yesterday before I left work it felt like my whole right side of my lower chest around my ribs were bruised and it hurt when I moved. it does not feel that way today. I was also diagnosed with GERD too.

First of all, it is sad to know what you had been experiencing lately. Based on the history you have shared, all these symptoms and discomfort may be due to GERD and this reflux can be triggered by the prime disease that is hiatal hernia as if it increased in size than before or causing stretch on diaphragm, it may present with such tearing pain. I would suggest you to have a chest Xray, and ultrasound and if needed a CT scan of abdomen and chest to assess the extent of hernia to assess for any need of surgical intervention.

If that is unremarkable it may need a repeat endoscopy to assess other pathologies like gastric ulcer disease or gastritis.

Meanwhile keep using antacids like omeperazole or pantoprazole or the one you are already using but it may need incrementing the dose upto 40mg daily and even 80mg daily after discussing with your primary doctor. A motility agent like domperidone may also be needed before meals.

Avoid fizzy drinks, spicy meals and sour foods. Avoid lying down for atleast 2 hours after foods and take small but frequent meals.
