Brief Answer:
These changes are irreversible

Detailed Answer:
Hello,
I haves studied your case and I think you are having seronegative arthritis. In this condition there is pain and inflammation in the sacroiliac joint. As you said that most of the tests are normal so you are having seronegative arthritis. Changes on MRI are also due to seronegative arthritis. You need to understand that sacroillitis is not cured completely but it can be controlled with drugs. So you need to take regular medicine jus like patient take for BP and Sugar. It does not healed completely.
Following steps are helpful.
1.Back muscle strengthening exercises and swimming.
2.Avoid forward bending or weight lifting.
3.Muscle relaxant like Myoril or Myospaz for symptomatic relief.
4.Steorid injection can be given by trained orthopaedic doctor. It gives good pain relief.
MRI showing sclerosis and some bone irregularities. So it is called subtle changes. It means that disease is under control and it is not going rapidly. Seronegative arthritis has better prognosis then Tb or Ankylosing spondylitis.
Thanks

Brief Answer:
It can be both also

Detailed Answer:
Hello,
After studying MRI report I think it is seronegative arthritis as there is erosion of joint, which can not be due to sport. Buy yes sports may contribute to these symptoms. You need to continue HCQ and swimming. Avoid very strenuous exercises.
thanks
