Question: Hi doctor I've been having issues with my periods for quite sometime. To be precise from October. I generally have a 4-5 days periods and the cycle duration is 24 - 28 days. But since October I've only been having 2 days of periods,but the cycle duration hasn't got disrupted. Now I am experiencing breast pains too, been having breast pains for the last two months, and it starts a week before my periods due date. Could you please let me know why I am facing such problems?

The cycles for 2 days bleed is fairly acceptable as long as your menstrual pattern is not disrupted, indicating that endometrium is being shed completely for another cycle to be initiated in just 2 days. Also stress induced rise in progesterone levels can often lead to decrease in menses. So rest assured.
Secondly breast pains a week prior to your menses is a part of premenstrual syndrome which may include nausea, headache, lower abdominal ache, mood swings and sore breasts. This PMS is indicative of the fact that your menses are to ensue in next few days and this is normal in most women. However if you find sore breasts troublesome, then you may use NSAIDS for pain relief and also hot fomentation to soothe the pain.

I had missed to mention a fact with my previous query. I had taken ipill in the second week of december., in the sense somewhere around December 11. But I did get my periods on the exact date that is 27th of December, but it was just for one single day. All this that I am facing right now, is this something to do will the ipill tablet that I had taken? Could you please clarify on this. Before taking the tablet on December. I had taken one in September too, all this short length period and breast pains had started only after that event. So, is this the side effect of the tablet?? What should I do to normalize my periods, I would want to get back to my normal period length of 5 days, what would you suggest me to do?

Sorry for the late reply,
IPill tablet actually can derange your menstrual period in the sense that your next few periods may be delayed and often scanty bleed shall occur. The breast pains are due to progesterone effect and as you mentioned that ipill was taken in december as well, these hormonal imbalances set in at your end can lead to these symptoms for 2-3 cycles but can be controlled with NSAIDS and hot fomentation. So please don't be worried.I would suggest you to just wait for 2-3 cycles and watch the pattern and don't take any more hormonalo emergency contraceptive pills.
If pregnancy is not desired then you may use regular low dose oc pills which can even regularise your cycles hence.

So doctor are you asking me to take up NSAIDS for the breast pains? Can anything else be done without taking tablets? Any other remedy that can be followed? And should I do a HPT, just in case to confirm that there has not been any conception? Because when i had taken Postpone 72, in September I never experienced breast pains, but now after taking Ipill in December i am having this issue of Breast pain, and in december also I had my periods flow only for one single day, and there was a thick brownish discharge before I started bleeding. What does this imply?

Hello,
You can undertake a serum beta hcg test to rule out any pregnancy conclusively, which i am sure shall show a negative result. Urine pregnancy test shall not be conclusive until 8 days post missed periods.
So the breast pains as in said are due to progesterone effect, a typical withdrawal symptom which is also a part of normal PMS in most women who have not taken any pills or so.
in your case it has been manifested post taking the ipill, which is quite normal and often expected for next 2-3 cycles when the natural hormonal balance is being disturbed and an early withdrawal bleed is initiated with the pill. The next 22-3 menses are likely to be irregular, delayed and often scanty to menorrhagia in some women, so the advise remains that one should watch her cycle for next 2-3 cycles and such breast pains or soreness should be treated symptomatically either with hot fomentation initially and if the pain cannot be tolerated then NSAIDS like TAB aceclofenac 100mg can be taken once a day, which can be easily prescribed by your physician.
Once the pregnancy test is negative, then you can safely wait for your regularise and get treated symptomatically for sore breasts and use barrier contraceptives to prevent further conception and avoid reuse of emergency contraceptives for next 2 months at least.

How can I get this serum beta test done? Can I walk into a lab and directly ask for a serum beta test or will I need a doctors prescription for that? And also doctor sometimes I feel like nauseating,but I don't vomit I just have that sensation for sometime. And sometimes I get headaches when I wake up in the morning. Are these symptoms of PMS as well?

Hello,
Serum beta hcg test is a blood test for estimating hcg levels which if are more than 5ng/dl shall confirm pregnancy. You can just go to the lab and ask for the test.
Secondly vomiting, nausea, headache, mood swings, increased frequency of urine, sore breasts , all are features of PMS actually and indicate a withdrawal in next few days once pregnancy has been ruled out.
The symptoms of PMS and pregnancy overlap , hence its better to rule it out first.
