What Causes Severe Abdominal Bloating Post A HCG Treatment?
Question: hiiiii I m dr XXXXXXX consultant pediatrician
I am a case of pcos
I am resistant to cc.this cycle I was given fsh for ovulation.i took hcg on day22 0f cycle wen there were 3 dominant follicles.my lmp is 14 july and hcg was taken on 2nd august
my upt was positive on 20th aug
I am prescrised duphaston 10mg bd
and took 2000iu of hcg after my upt was positive
for the last 6days I had severe bloating sensation .hence I underwent usg abdomen which showed g sac
and enlarged ovaries(right ovary measured 8.5*2.4cm with a simple cyst measuring 6.2*4.5cm
left ovary is 6.5*5.2cm with a cyst measuring 4.3*4.3cm.no free fluid in abdomen
my gyn is suspecting it to be mild form of ohss...iam asked to discontinue hcg and continue duphaston.
my q is -is oral duphaston as effective as endogenous progesterone to retain my pregnancy.....is it fine if I stop hcg?
and will these cysts continue to grow if I take hcg?
and my abdominal bloatin has come down wid h2 blockers...........
can u pls give me a plan of further management as I cannot access any infertility specialist nearby

1) You have developed OHSS. it is good to stop hCG injections immediately.
2) OHSS might increase further for another few weeks and might lead to more pain and distention due to pregnancy.
3) Have more protein in diet including egg white and pulses, meat if non vegetarian.
4) Drink more water.
5) If you feel worst, see an IVF center nearby immediately.
6) Duphaston is effective.
