Question: Doctor,I m suffering from severe anxiety followed with depression and fluctuating blood pressure.i m scared of living alone with endless negative thoughts.i feel as if I would die.occasionally I take wine also as it eases me off.my doctor knows about it.he has prescribed me medicines which doesn't interfere with it.i m taking medicines since two years but all in vain.i m not getting any relief.i want to quit medicines but I can't.i m married with six years old child.i m really depressed and living very pathetic life.i faint often,chills,restlessness,etc.if possible can u provide me your contact number.my email I'd is ... I appreciate your efforts for medical consultation in so much distress.

Important aspects of your query are:
•     35 year old female
•     Known case of anxiety disorder with secondary depression.
•     Complaints of: Poor response to treatment.

From, your query it appear that you have multiple anxiety and depressive symptoms which are affective your life.

Normally anxiety patient respond well to treatment. But you are not. In that case there is need to find out cause of poor treatment response.

Some time patient (40% cases) did not improve with treatment like in your case, in that case one of the following steps are used:
-Checking compliance to treatment: Take medicine regularly: as medicine used to show effect after 4-6 weeks of continuous use.
-To rule out any medical condition leading to existing symptoms
-Detection and correction of vitamin deficiency (especially vitamin B12): so take vitamin supplement and fruits/milk in diet.
-Screening of thyroid dysfunction: Get serum T3/T4 and TSH level done
-Monitor Blood pressure
-Increase the dose of current medicines
-Change of existing medicine
-Addition of other medicine

As you already have multiple anxiety and depressive symptoms, so do not stop your current medicines.
