Microangiopathy means involvement of small terminal branches of blood vessels supplying in brain.
It can be due to age, hypertension, dyslipidemia, blood sugar and may be vasculitic process.
Some amount is commonly found with age in most person but if it's more then evaluation by blood investigation required.

You can get blood pressure measurement, blood sugar, liver and kidney function test, lipid profile, vitamin B12, ANA profile and compliment levels.
Headache seems not to be related to this.
