Brief Answer:
Reflux

Detailed Answer:
Dear Mr XXXXXXX
Going through your history and part report of the endoscopy you have undergone it appears that you have Reflux or GERD.
Would suggest you to avoid fatty food, spicy food, tea and coffee in moderation and take a PPI (omeprazole, ilaprozole etc.) with a prokinetic (lesuride). Your doctor can suggest you the same.
Antibiotics in yourself probably accentuates the reflux which gives you thesesymptoms.
Follow the diet advise and it should help you.
Take care
