Question: I am finding myself backed into corners trying get my health back in order. I noticed a serious decline about 8 years ago and it has gotten significantly worse. It started with stomach pains and then I wouldn't eat for a few days or weeks and it would go away. I then developed direahea that has not gone away for 8 years. Then came fatigue which as of the past year has gotten severe. The last set up symptoms have been severe abdominal pain and bone and muscle pain. I've also developed petechiea.

Since 2009, I have been diagnosed with various conditions, treated for some, and dismissed on others. I had a previous doctor diagnosis the abdominal pain as PCOS without any confirmation of the disorder. This prove incorrect within the past two years by my current gynocologist. In 2009, I was diagnosed as having fibromyalgia with zero testing and no treatment even though I was having severe muscle pain.

My current PCP diagnosed me with fibromyalgia, offered no treatment and referred me to a rheumatologist. I have been struggling the past year with crippling fatigue, bone pain, muscle pain, and abdominal pain. The rheumotologist concluded last week that given my lab results there is no way that fibromyalgia explains it. She wanted to me to follow up with a hematologist and the hospitals hematology department permits a self referral, however, as of today they have refused me twice since August.

I am currently waiting for the rheumotologist to follow up with me tomorrow due to the still elevated ucic acid levels on my most recent lab work. I have been dealing with this horrible pain and fatigue now over a year. I am barely functioning enough to make it through the day. I have lost my life to what this is and beside the rheumatologist I have been dismissed. I am tired, in pain, and frightened because I do not know what is going with my body or what my future holds. I am struggling financially because I have been referred not only to the rheumatologist but also to a spine surgeon, neurologist, sleep clinic, and have been prescribed a host of medications that have done nothing.

I am quite well aware of what things have been ruled out. However, I do not have a diagnosis and I feel is though I am running out of options. As of right now I have been experiencing severe pain in my right side that radiates to my ribs and through my back, pain right under my ribs on the left hand side, sore ribs, nausea, and low grade fever. However, if I see my PCP, I get dismissed. The ER never gets me anywhere but MRIs, CAT scans, and nothing. I am at a total loss of how to deal with the pain. No one will help because I have no real diagnosis. I don't know where to turn at this point. How do I get a physician to actually hear me and understand how much I want to be well again?

I have also gone through your previous quries.

I completely understand your frustration and your angst, suffering without a diagnosis is quite horrifying to say the least.

I understand that you have been subjected to many investigations multiple times to find a cause, (it is a reasonable guess considering the fact that you have been to numerous specialists), so it would help me understand your condition better if you can upload all the reports till date.

The silver lining in all this mess and confusion is, if you have undergone many tests (particularly MRI and CT scans) several times, and they have failed to reveal anything significantly abnormal except uric acid levels, it is highly likely that you do not have any major structural cause for your symptoms (fibromyalgia syndrome is still a possibility in such a situation as there is no detectable structural cause). The exact mechanism of this disease is not understood very well, but the brain interprets the signals differently so you actually feel debilitating pain.

Fibromyalgia can be treated well with medications like Duloxetine. This needs to be looked at again once uric acid levels are in control.

Most of your symptoms can be related to High Uric acid level.
I think your Rheumatologist will put you on Anti gout medications in your next appointment. You have to limit your protein intake also.



Regarding Fibromyalgia, it is a diagnosis of exclusion. ie when you rule out all other causes of your symptoms only then you make a diagnosis of Fibromyalgia.
Drugs like tramadol are of great use in situations like this.

Tramadol can help you in relieving pain symptoms. It may not make the pain go away completely, but it definitely decreases it. This will help to get a sound sleep.

I would suggest you to follow up with Rheumatologist. Something will turn up. Keep your faith.
