What Causes Severe Pain And Swelling In Lower Cheeks Of A Child?

Hes having the doses of the prescribed medicines since Tuesday Afternoon but the pain and fever is still there and the cheek has swollen more than before. And as for the fever and pain, I am giving him Ibugesic plus 4/5 ml four, five times a day because the pain gets very uncomfortable for him and goes about his normal activities only if ibugesic is given. After the effect of the ibugesic wears off he wd seem a lil upset and then wd start feeling the pain again. This evening (May 5th) I went to the Dentist again and asked if the Antibiotic was working well to which the dentist said that antibiotics wd atleast require 3 days to start its work and after that the pain and swelling will ease. Seeing his swelling cheek, he even prescribed 2 more medicines which are; 1) Tab Enzomac (Half a tablet twice daily) and 2) Septilin Syrup (Himalaya Product) (5ml, twice daily)
Now I am worried as hell since he has had 5 doses (2 and half days) of every medicine the dentist prescribed and till now the pain and swelling is there. I am also worried about the side effects of ibugesic plus. Since the fever and pain is persistent, I am having to administer the ibugesic to him. I am giving him ibugesic pluc every 6 hours.
Will my kid get cured of the infection? The doctor has also said that till now abcession is not there. But I can see the swelling has increased and looks like hes got double chin. Is the fever of concern? How soon you think the pain and fever as well as the swelling wd recede? Please give me a good advice Doctor...I am very much worried.

Thanks for your query. I have gone through your query. As far as the swelling, fever and pain is concerned, it is because of the tooth infection that has resulted in abscess formation which has spread to involve soft tissues resulting in cellulitis. Since you have already started with Medications like Zocef and Ibugesic, you can continue the same but stop rest all medicines particularly Septilin because this will spread the infection rather than limiting it.
Nothing to be panic, consult a pediatric dental surgeon and get the decayed tooth treated immediately (access opening and puss drainage), the swelling and pain will come down once the pus is drained. Consult the dental surgeon as early as possible because if the infection spreads further it will lead to difficulty in breathing and swallowing.

The infection has spread to the soft tissue because of the drugs which you have taken previously. Nothing to be panic, immediately consult a pediatric dental surgeon and get the tooth treated. Upload the images of the swelling so that it will easy for me to give better diagnosis. The swelling takes time to come down, probably it may take 5-7 days. The chances of mumps is very less because mumps involves parotid glands not submental or submandibular glands.

Also, I gave my kid another medicine "Septilin Syrup (Himalaya Product)" for only once (5ml) yesterday night.
I just went to a different dentist (located in a different city) with my kid today morning and he just changed the antibiotic and asked me to administer 3 doses of 5 ml a day (Syrup Clavam) and prescribed another tablet called Flagyl 200 (may be my spelling is wrong).
However this dentist said theres abscess and he just pricked it with a needle and drained out about 2.5 ml of the blood from that part. How come the previous doctor yesterday said there wasn't any abscess till yesterday evening. Abscess can't grow overnight what I know....isn't it right?
The different dentist whom I visited today asked me to check today if the swelling further increases. he asked me to visit him tomorrow again and if needed he wd ask me to admit my son there for treatment. But as soon I heard this I got frightened and I am begging you to assure me if my son will get cured from here. What might be the procedure if hes admitted? If this thing very complicated. The new dentist from the other city whom I went to today for the second visit first checked my son's swelling and said theres a little improvement he could see. However he consulted a XXXXXXX surgeon in my presence and that surgeon first checked if puss could be sucked out/drained out by inserting an injection on the outer side of cheek however he found out that there was no puss inside so he cannot do an incision on the cheek as it wd be unnecessary since the puss hasn't formed yet. However he maintained that puss could be formed if antibiotics are not properly given. So after consulting with each other the Dentist prescribed " CLAVAM INTRAVENOUSV 500mg thrice a day for 4 days along with Betadine Mouth Wash and Ibugesic Plus and Tab Flagyl 200 and Bifilac Syrup (Probiotics). Will my Son get cured Dr. I am very worried. Please help with your suggestions.

Thanks for your reply. I have gone through the images. As per the images it looks like cellulitis.
Nothing to be panic, you can continue with the intravenous antibiotics like clavulnic acid and amoxicillin.
If it is an abscess, it can be drained. Once the abscess becomes cellulitis, it cannot be drained. So it has to be treated with a course of antibiotics. Once the swelling subsides, we can treat the causative tooth. The infection will subside in 3 days.
Some drugs like serratiopeptidase will spread the infection if it is administered while treating an abscess.
Please do not keep on changing the dentists and antibiotics. Stick on to a single dentist who has done MDS in oral maxillofacial surgery and get it evaluated. Repeatedly changing antibiotics may result in development of resistance and the infection will not subside.
Nothing to worry about your child. He will be alright in 3 days. Be bold.

I had to change the dentist as the previous Dentist prescribed "Zocef Dry Syrup" which didn't cure the infection. Also on Tuesday when I first visited the 1st Dentist ; he told me there wasn't any abscess yet so it was an infection according to him and so he prescribed Zocef 2.5 ml first then 5ml on the 3rd day. However all that didn't seem to control the swelling so I went to a different Dentist who immediately prescribed Clavam.
Now Dr. Dir, I wd want to know what might be the procedure to control the swelling apart from the intravenous administeration of the Antibiotics? Have you ever come across a child like mine whos 5.5 years of age and had similar situation like my child? Can antibiotics cure such completely? Since the puss isn't there when checked by a surgeon yesterday; do you think puss can still form? How long really intravenous antibiotics take to bring down the swelling. I am still giving him ibudesic plus (Every 7/8 hours) because the pain is there and the fever (sometimes mild). Can I give him ibudesic plus till the swelling gets down?

The drug which might have increased the swelling can tablet enzomac which contains a proteolytic enzyme.
The swelling will come down by antibiotics then you can get the tooth treated. There is no other way to reduce swelling. In cellulitis also pus will be there but it will not be localised. so it canot be drained.
We have seen many cases like your kid. it is a common condition.
Nothing to worry you continue the medicines like amoxicillin and clavulnic acid. The infection will subside in 3 days, then get the tooth treated.

Currently hes taking the following meds;
1) CLAVAM IV 500mg (for five days)
2) Ibugesic Plus Syrup
3) Bifilac Probiotic
4) Flagyl 200 tablet (Twice a day)
5) Betadine Mouth gurgle (4 times a day)
6) Bruvam-L Syrup (Lycopene with zinc, Selenium and minerals)
This medicine (BRUVAM-L) was prescribed by the previous (First) Dentist and I had discontinued it since tomorrow. Could it be given again now or I should avoid it?

The fever is common when there is cellulitis. Nothing to worry.
You can get a radiograph done like OPG(Orthopantomograph) to check which tooth is involved. I suggest you to get the infected tooth treated now since you have completed the course of antibiotics. You can continue the medications which you have told in your previous reply for a course of 5 days. Bruvam can be continued, it is a just a supplement. Once you get the OPG done you upload the images, i will let you know what has to be done.

I gt the OPG done on Friday but had to leave the report at the Diagnostic Centre as the Dr. at that time asked me to leave it there so he could check the report during night (Friday) and inform me what to do later. On the same night (Friday) I received a phone call from him saying that he wd have to extract the tooth (Molar-Lower Left side) so we wd need to go to him on Monday.
However I am very hesitant to go abt it without a suggestion or advice from you of course. I am all the more worried since its a milk tooth and a molar tooth and my kid is just 5 and half years old. The swelling has subsided to more than 75% but the Dentist said extraction needs to be done now as the tooth itself is an infected tooth and we need to prevent such infection in the future. So please suggest me Dr. if its safe to extract a tooth like this (Theres a huge cavity to all the way down in it). I am asking because hes just 5 and half years old. Also the dentist said that my son should be given 5ml oral anibiotic twice a day since a day before visiting his clinic for extraction. Please understand that I need an advice from you urgently as today is sunday and I wd be needing to go visit his clinic tomorrow morning.

If the tooth is very badly decayed and not in a condition to save, then it has to be removed or extracted. Once the tooth is removed, then the pus can be drained through the extraction socket.
If sufficient tooth structure is not there, then it has to be endodontically treated.
If possible upload the images of the orthopantomograph so that I can tell you whether the tooth has to be removed or saved.

Apart from that, I wd like to really know is it very complicated to remove a molar tooth from a 5 year old child?
Also, will there be any complications now if the tooth is removed since some infection may be there as its been just 3 days that the antibiotics' treatment course is completed. I am worried because theres is a general assumption among common people that a tooth shouldn't be removed untill infection is healed so that cancerous growth doesn't take place. These theories are making me weak in the mind.
Or do you think I should wait some more days till the infection is healed completely?

Ideally the molar milk teeth will shed at the age of 12 to 14 years. Since the tooth is infected and sufficient tooth structure is not remaining(means there should sufficient crown structure that is the visible portion of the tooth to do a root canal treatment or endodontic treatment to save the tooth) then it has to be removed. If the crown structure is sufficient then it can be saved by doing root canal treatment. Ideally speaking we need to save the tooth till the age of 12 years but if the tooth is not in a condition to save then it has to be removed.
Removal of a milk tooth is a simple procedure which is done under local anesthesia. Nothing to worry about cancerous growth and all. There is no possibility of cancerous growth arising from an infected tooth. Once you complete the course of antibiotics for 3 or 5 day's you can get the tooth removed. Be bold it's a simple infection. Do not postpone the procedure, get it done as early as possible.
Upload the images of the orthopantomograph so that I can clearly tell the treatment plan and I can explain you better about the problem with the tooth.

You need to get the tooth removed but I want to see the orthopantomograph before you go for extraction. You can start with oral antibiotics before extraction. The antibiotics are given during extraction since the procedure is a minor surgical procedure and the socket will be exposed to oral cavity following tooth removal. The antibiotics will prevent further infection and also resolve the previous infection. Upload the image of orthopantomograph. I hope I have answered your query. Take care.
I am sending the pictures...please find them attached. The latest update is that today the tooth was extracted without any problem. My question is that when will the permanent tooth in its place will grow up. Will the growth of the permanent tooth happen now in its place or it will grow after a couple years?

Still the permanent premolars have not formed. It might take 2 to 3 years for the teeth to erupt. There was sufficient tooth structure left in the milk tooth. we could have tried doing root canal treatment. Nothing to worry now. wait for the tooth erupt.
