Detailed Answer:
Hello XXXXXXX and welcome to HCM.
As an Urologist, i can fully understand and share your concern.
After going through all your reports, you don't really don't need to have so much anxiety.
I shall answer your questions, one by one.
1. Penis always shifts to any one side normally, and it never causes any difficulty in having intercourse.
2. You should have no problem in having intercourse, due to your present symptoms. There's no need to worry about it marital planning. Go ahead confidently. This is noted with many people, having similar symptoms.
3. Testicular torsion is an acute condition, which requires urgent surgery to correct it. This is very easily diagnosed and very different from your symptoms. The doppler scan can detect it very easily.
4. Epididymitis is another acute condition, which's very different from your symptoms. The doppler ultrasound scan can easily diagnose it.
Complete the course of Levofloxacin, you've started.
Hope all your doubts are cleared.
You're welcome to clear any other doubts you may have.
Wish you good health.

Detailed Answer:
Hi XXXXXXX
Yes, for scrotal ultrasound, a doppler scan is needed, to detect blood flow into and out of both the testis.
This will, give vital information regarding the following :
1. Varicocele.
2. Epididymitis.
3. Scrotal congestion of whatever condition.
4. orchitis.
5. torsion testis.
6. testicular infarct.
7. Testicular vascular supply, and many other conditions.
If doppler scan of scrotum has not been done in your case, kindly get it done, as it may give an idea of the cause for your pain.
If possible, report as quickly as possible, to a surgeon or Urologist, once the testicular pain starts.
Hope to see your doppler scan reports.
Wish you good health.
