Question: Hello Doctor, The problem I am mentioning below is about by auntys health issue. She is in her 50s now. She had her rheumatoid arthritis problem for past 5 months.. Now for the past one week she is suffering from severe pain during night on the inflamed right shoulder while sleeping... sir, she had T-sazz 1000mg (morning and night),oncotrex weekly once for past 3months now the doctor advised to take hydroquin (morning),T-saaz and T-Ezcat during (night)..whether the newly prescribed tablet can be taken for treating Rheutamoid arthritis....i have to know the details regarding hydroquin,sazz and ezcat..Please guide us which is the best medicine without side effects. Blood test report SERUM ANTI C C P (cyclic citirullinated peptide) IgG Result= 0.90 RU/ml less than 5.0 (ELISA METHOD)

1.Hydroquin 200/400 is a drug used in rheumatoid arthritis (RA), which will show effect after 4-6 months of use. It is one among the so-called disease modifying agents (DMARD). It is not intended to kill or reduce the pain in joints, instead it reduces the disease process of RA. It is generally safe, but long term use may damage eyes. Regular eye check up before and 1 year after initiation of this drug is mandatory.
2. Saaz is a sulfa drug, which is used as another DMARD, having similar effect. It can reduce bone marrow function and can cause oral ulcers on long term use.
3. I do not get your "ezcat", please correct if there is any spelling error.

sorry sir the spelling error the 3rd tablet is T.Ezact 90mg .in the tablet cover it is mentioned this drug should be used with caution in patients suffering from coronary heart disease/cardiovascular disorder..she is having pressure sir use to take seloken XL 25 ,cilnipar in morning..yesterday night she had some heartbeating problem..whether it is advisable to take this tablet in night time or to reduce the dosage for seronegative arthritis.How many months sir this tablet should be taken any sideeffects.

Ezact90 is etoracoxib, a pain killer used to reduce pains (immediate effect). it is a safe drug, even though studies have shown some risk in heart patients, this should not be a concern. Dose reduction is not needed, as your aunt is just a hypertensive.
As i told, RA is a long term disease which needs treatment for prolonged periods of time. Periodically, patients go into remissions (disease free time) when, one can reduce the doses of all these drugs. similarly, there will be relapses too (disease flareup time) when they need increased doses. Finally, the disease may become quiescent (in some people) after some time..may be after 60-65 years of age, when we may stop all the drugs. It is highly variable from patient to patient. A good followup with a rheumatologist or a physician will reduce the development of complications and joint deformities.
