What Causes Severe Pain In Scrotum?

Question: Hello.
I have been having severe pain on my scrotum for about 4 weeks. Not the testicles (internal), but the actual sac.
I have been to a doctor, and the ER twice, and had an ultra sound done (ultrasound showed nothing wrong) The doctor treated me for epididymitis, which the ER said was not the issue. Both diagnosis from the ER said nothing was wrong and was most likely an inflammation. I had been taking Naproxin, which did nothing.
I started using a Anorectal Cream which contains 5% Lidocaine which helps tremendously.
Today while inspecting the sac and noticed there was a patch that was a lot "smoother" then the rest of the sac. The area is white (compared to rest of the sac which is red) and hair is no longer growing there, this is also the area where the pain is. The pain itself is very severe.
Personal info, I'm 35 years old, 110 pounds, Diagnosed type 2 diabetic (2 months ago)

Thanks for your query ,based on the facts that you have posted it appears that you have severe pain in scrotum and ultrasound scanning has revealed testicles and epididymis to be normal .

The pain and yellowish patch that has developed on scrotum is mostly due to inflammation of soft tissue (Cellulitis) which is very common in people with uncontrolled diabetes ..

You need to consult qualified General Surgeon or Urologist for clinical examination and get following basic test done to help to establish diagnosis
1) Haemogram
2) urine routine and culture
3) A piece of necrotic tissue for culture and sensitivity .

In the mean while take broad spectrum antibiotic like Augmentin and anti inflammatory medicine like Diclofenac twice daily later on switch on to appropriate antibiotic as per culture report ..

You may need to do debridement of the infected tissue followed by daily dressing till it heals up completely .
