Question: I have severe stenosis and severe pain in my left shoulder and upper arm. I am not able to do my work or shop because I cannot stand or walk more than for more than 5 minutes. Two masses have been found on my liver. I have had MRI and CT Scans and the doctors are still searching for answers. I am disabled by pain. Do you have any suggestions concerning my problems? XXXX

Brief Answer: Try Carbamazepine+Vitamin B12 insteadof gabapentin Detailed Answer: Hi Mrs. XXXXXXX Thanks for posting your query on this forum. I can fully understand the agony that you are passing through. However, the saying that "permanent solution to a problem is to nip it in the bud" holds good for treatment too. The harsh reality is that the severe, excruciating pain and severe immobility will persist till the compression over the spinal cord and the emerging nerves is removed by surgical dilation of the spinal canal. You are already on lot of nerve-pain killers, which can reduce the pain by a limited extent only. Perhaps, Carbamazepine (an anti-epileptic drug) in maximum permissible dose, may be tried in place of Gabapentin. In addition, Vitamin B12 in high doses (1500micrograms/day) will also help to some extent. Consult your doctor and apprise him of my opinion. I am certain that he will agree with me and substitute Gabapentin with Carbamazepine in appropriate dose, as advised. Hope I have addressed your concerns to your complete satisfaction and you find my response helpful and informative. I will be happy to answer further queries, if any. Study of the MRI/CT scan reports may provide me some valuable information. you may therefore upload the reports for my perusal. Fond regards and have a nice day, Col (Dr.) Rakesh Karanwal
