Brief Answer:
Can be an intestinal obstruction.

Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Female/62 - Lap choley with complications - corrected by open method 2003 - scar becomes painful from time to time - Now- last few days - pain in abdomen - upper and lower - severe - tenderness - pain on sneezing - feels very bloated - no BM at least 2 days - tried Colace stool softener - lots of water - decreased appetite ...

The most probable causes I can think of with the history you have provided are:
Intestinal obstruction.

I would advise you the following:
X-ray of the abdomen in standing position can be the simplest and cheapest investigations to know whether there is an intestinal obstruction, and if present can also tell about which part is obstructed, dilated and so on.
Additional investigations of Ultrasonography of abdomen, Contrast enhanced CT scan of the abdomen help more.
Get the tests of blood, urine and stool also done.
All these things with clinical evaluation and an actual examination can help to get the correct diagnosis to have a plan of management.
All these points in view of the past history of complicated surgery and re-surgery.

I hope this answer helps you to get an early diagnosis and have proper treatment to have a cure. Please feel free to ask for further relevant queries if you feel that there is a gap of communication.
