What Causes Severe Penis Pain During Masturbation?

It may be minimal inflammation of epididymis.
Detailed Answer:
Hello XXXXX and welcome to HCM.
As an Urologist,i can understand your concern.
You've taken a wise decision to see an Urologist, because that makes diagnosis easy. There may be an inflammation over the cord, called epididymitis.
The next best thing you can do is to send a photograph.
That would give some idea,of what's the smelly dirt particles, you're referring to.
As far as the testicular pain is concerned, you'll need to do a scrotal doppler scan.
That will show changes in the testis and the cord,so one has an accurate idea.
According to the diagnosis,an expert advice can be given.
Till then, use a scrotal support,so that the scrotum is lifted upwards.
That will reduce any inflammation in the testis area, and pain will be reduced.
You can take an analgesic tablet,if it's paining severely.
Avoid masturbation till you see the Urologist.
Maintain good penile hygiene at all times.
Avoid moisture over the penis.
Wish you good health.

Photo suggestive of infection.
Detailed Answer:
Hello XXXXX,
It's good you've sent the photo and seen an Urologist.
A direct examination is never the same as seeing a photo.
The photo is very suggestive of an infection.
In that case, the treatment you've started is proper.
However, the treatment may be needed for about a week.
You'll need to see him after 3 days,if the there's discharge over the penis.
There's nothing to worry about.
Avoid masturbation during course of treatment.
Maintain good penile hygiene and avoid moisture over the penis.
Wish you good health.

It's only to be sure you're not a diabetic.Complete the antibiotic course.
Detailed Answer:
Hi XXXXX,
You're right that it may not be diabetes.
But this kind of balanitis changes, are common in diabetic people.
That's the reason to be sure,it's not diabetes. That's all.
If you've no family history of diabetes,it's very unlikely to be diabetes.
Take the antibiotic course and maintain good penile hygiene.
There's nothing to be worried about.
Wish you a speedy recovery.

Allergic reaction to condom is a distinct possibility.
Detailed Answer:
Hi XXXXX,
If you've had sex with multiple partners,it's best to get STD panel tests done.
There are very mild changes of balanitis seen.
Continue the antibiotic for another week.
Stop masturbating or having sex till full recovery.
Possibility of an allergic reaction to the condoms used,is also there.
Wish you well.
