The bilateral avascular necrosis of both the hip joints does not need an active intervention if it has not yet collapsed. Since the necrosis is an old one, revascularization is not possible.
Once the hips get collpased due to old age, the hip replacement is a good option. In case the pain is intense even then a replacement can be considered. No active treatment is needed if there is no pain.
