What Causes Severe Tooth Ache After The Molar Teeth Filling Has Fallen Out?
Question: I need to speak with a dentist. Severe pain. Filling placed in Rwanda has fallen out. Dentin exposed. Using benzocaine. Worried about methemoglobinemia.
Live in the US, recently got a job with dental insurance, but only pays 50%. I imagne I am going to need an expensive procedure if it is irreversable pulpitis. Having to use benzocaine so often I am sincerely concerned about methemoglobinemia. Can I go to a physician for a stop gap measure until I have enough money to see a demtist? What do you recommend?

I have gone through your query and can understand your concerns..
As per your complain if you have Irreversible Pulpitis then taking medication or getting the tooth filled without Root Canal treatment will not resolve the problem completely, rather will worsen it by causing pain and swelling..
So going to a Physician makes no sense and you need to see a Dentist only..
Secondly I would like to know that as you are worried about Methemoglobinemia, so are you suffering from the same or worried that you will get it as a side effect of benzocaine or dental treatment???

I have been having to use much more than the maximum daily dosage of topical Benzocaine 20% in ordsr to keep my pain under control.
Im also taking ibuprofen 800mg every 8 hours. Do you have any additional suggestions for pain control until I can see a dentist?

As per your query, Methemoglobinemia is a rare side effect of benzocaine use but cannot be ignored..
Secondly using benzocaine topically can provide only temporary relief and is not at all a permanent solution for your problem.
Similarly taking Ibuprofen will relieve pain till it is effective and as the effect will wean off pain will recur..
So in tooth related infections, the best and the appropriate solution is to get the tooth treated..
If tooth can be saved, then root canal treatment will help in permanent pain relief..
In case of gross decay, extraction has to be done..
If as of now pain is severe you can take Ketorolac or Tramadol..
Applying clove oil or biting over a clove can also provide relief as it has sedative properties..
Do cool compresses over the cheek..
Avoid chewing from painful side and also take a soft diet..
