Brief Ans- Pain, central upper abdomen.-Post Cholecystectomy?Under Investigation
HI,Dear,Thanks for your query to HCM.
I studied your query and reviewed those facts in contex of your health concerns.
I found that further information is needed on the following points of your query-
1)male, 5'5, 182 lbs, 48 years old. Had a gastric sleeve a bout 2 years ago, and am still down 65 lbs. -
-What were the complaints 2 yrs back for which you had done this surgery?
What was your job work/dietary habits/and life style 2 yrs back?
2)I had my gall bladder removed (which had many many stones) about 3 months ago.
-For which complaints gall bladder was removed?Whether any of the complaints is still there?
What was the job work / dietary habits, excesses of alcohol/and other medical illnesses since 2 yrs back?
any change in these lifestyle factors, after gastric sleeve was done?
What complaints were relieved after cholecystectomy?
Was there any back pain with central abdominal pain 3 mths ago before cholecystectomy?
3)I've had severe pain in my middle abdomen, which also radiates to my back.
-After how many days, did you start getting this severe mid-abdominal pain with back radiation?
Did it start after any food excess? How did it start and when did you get this mid-abdominal pain
4)I've had an MRI to make sure more stones hadn't gotten into my common bile duct, and I'm clear. The doctor also told me that kidney functions from blood test were ok, and that there wasn't any evidence of pancreatitis at that time either.
5-I'm still having significant paint, central upper abdomen. No amount of antacids/pepto...seems to help.
How frequently do you eat outside food? Do you have travelling job every week?
How frequently you have spicy, oily food?Any tea, coffee, tobacco, alcohol, smoking excesses you have?
Please update above information and its status-
a- 2 yrs back before gastric sleeve operation/
b-after gastric sleeve and before cholecystectomy?
c-and after cholecystectomy in last 3 mths?
6-Any ideas?
Please update me detailed information as asked above.
As without having this information,it would be very difficult for any doctor for that matter, to reply on the causes of the Central Abdominal Pain with back referral.
So to tell you the causes of the central abdominal pain with back referring,it is very important to update this information from your end, soon.
Awaiting above information and any further query, which could be reviewed in the next interaction session.
Would love to help you out, even though it is very challenging task,in the absence of the above information.

Alcohol consumption has increased moderately, all else has stayed the same since my gall bladder surgery. I had referred back pain prior to the cholecystectomy as well.
I drink a lot of coffee/tea, but always have.

Hi,thanks for the updates.
I reviewed your query meticulously in context of the query facts and in context of your 6 no question -I'm still having significant paint, central upper abdomen. No amount of antacids/pepto...seems to help. Any ideas?
Your query has been resolved partly as many factors of your query opened out.
This expert knowledge has explained the query by My Ans to -Why the mid-abdominal pain is still there with tons of antacid and proton pump inhibitors?
Dear, you have all these complex entities, only due to the –Travelling Job with Outside frequent food and with increased alcohol lately and frequency of coffee/tea which always have, with lots of stresss/lots of travel, which was and is a major contributor to your mid abdominal pain. This pain of Gall bladder stones with cholecystitis was refferred to back to scapula, as the nerve supply to removed gall bladder is from T7 level.
Besides this though your MRI has ruled out any redundant calculi left in CBD,you seem to be suffering from PCS(Post-Cholecystectomy Syndrome) still in 4 th month after cholecystectomy-which is of non-biliary nature and related to the neural damages of sympathetic(pain related)in nature with resultant back referred pain –which is phantom pain of removed inflamed gall bladder.
Hope this explains the cause of your sever post operative pain in the mid-abdomen.
Remedy-
If you change your lifestyle and dietary habits, You have options to come out of this pain and anti-spasmodics / Proton Pump Inhibitors dumping without any relief ,causing more anxiety and depression from this cycle.
I would suggest to get MRCP (Magnetic Resonance Cholangio-Pancreatography)non-invasive study done to rule out the missed out stones at sphincter of odi, causing biliary dyskinesia and pains and dyspeptic pain with it.
Awaiting any further query, which could be reviewed in the next interaction session.
Would love to help you out, even though it is very challenging task of post-operative complex pain.
Good Day!!
