What Causes Sharp, Burning Abdominal Pain, Constipation And Bowel Incontinence?

Question: Hello.
I've been experiencing sharp, burning abdominal pain for the past 5 days that has be steadily getting worse and worse. Today I've been in excruciating pain all day. Best way to describe it feels like I have acid in my abdomen.
Also over the last month I have been having increased bowel problems. I've been having episodes of constipation when I'm unable to have a bowel movment for up to 4 days followed by days with 10+ bowel movements. This has gone hand in hand with more and more occurrences of bowel incontinence. I'm now at the point that as soon as I feel one coming it is already to late and in my pants.
I don't know if the 2 are in anyway connected.
Brief background and history. Diagnosed in December 2013 with advance liver cirrhosis/end stage liver failure. Sever ascites. 3 hernias (umbilical, inguinal and hiatal), inguinal contains a large amount of my bowels. I'm on diuretics and lactulose.
Any ideas or suggestions would be much appreciated. Thank you for your time.

Hello,
I had gone through your question thoroughly and i can understand your concern very well.
As per your history, you are already suffering from liver cirrhosis and hernias, the pain you are feeling might be because of obstruction or strangulation of inguinal hernia or may be due to secondary infection in abdomen due to ascites.
So i would suggest you to drain your ascitic fluid fully and if the severe pain persists, then you will need to consult your physician and rule out any kind of obstruction in hernia or any infection in peritoneum. I would suggest you to go for CBC, Blood Creatinine and ultrasound of abdomen.
Lactulose increases the bowel movements and that might be the reason of increassed and uncontrollable bowel movements. But at the same time, lactulose is also important for liver cirrhosis to increase defecation. You can adjust the dose of lactulose after consulting your physician.
