This may indicate torsion of the testes.
Other possibilities are epididymo-orchitis or varicocele. But the former usually causes a constant pain rather than on and off pain, and the later a dull aching rather than sharp pain. So torsion is an important possibility here.
Unlike other two conditions, torsion is an emergency and need urgent intervention to prevent vascular cut off and gangrene of testes. Manual derotation can be successful if presented early. If failed, this may need surgical correction and fixation which is a minor procedure done under local anesthesia.
So kindly reach ER without delay. After examination, you may be asked for a USG doppler of the scrotal sac.
