There can be many causes for sharp pain under the left arm:
Heart problem has to be immediately ruled out by an urgent EKG and cardiac check up. Get an assistance to visit the ER to get the vitals checked and get an EKG...

If the EKG and heart parameters are normal, the other causes have to be thought of ; and they can be :
muscle spasm
developing cellulitis or an abscess.

The Doctor in the ER will also check you for the other probable causes for the pain under the left arm..
