What Causes Slurred Speech When Diagnosed With Granuloma?

Question: Hi , My father age 62 ,weight 58 kg. he is a diabetic patient and 3 weeks back his speech started slurring. after admission to hospital in MRI doctor identified as granuloma. doctor gave him Dexamethasone as injection for 7 days and later medicine for 5 days. during this his sugar has increased a lot.
we consulted other doctor and he stopped Dexamethasone and started insulin injection. he was ok for last week and today I can feel slurred speech again (slip of words).
mRI report ,prescription of both doctor is enclosed.
since more than a week he is on levepsy 500 mg twice and insulin injection.
please advise

Hi dear,
I have gone through your question and understand your concerns.
Slurred speech is unlikely due to granuloma as it might have been present there for a long time.
Deranged blood sugars, transient ischemic attack or cerebral edema caused by sudden enlargement of the granuloma can be the cause of slurred speech.
Dexamethasone is given to decrease the cerebral edema and inflammation, thus improving the symptoms.
I will suggest you to consult a neurologist so that proper examination should be done to rule rule out any associated motor or sensory deficit and get futher treatment timely.

hi ,
we have shown him to neurologist Dr XXXXXXX Hasti. as per him follwoing medicines are to be taken .. levepil 500 mg and melzap .5 mg in case of emergency . prescription is attached.
now his slurring problem is increasing day by day .. is it normal.

The treatment is good and should be continued.

The lesion on MRI is located at the site which controls speech of a person and so there is involvement.
Mild fluctuations can occur for few weeks.

With time the edema subsides and lesion decreases.
Increase in dose or additional drug can be required if seizure reoccurs.

Hi Doctor,
requirement of Melzap .5 mg has become twice or thrice in a day . it's been taken when slurring in speech increases too much . after taking melzap .5 mg it becomes normal .. Is it ok .. or do we need to do something.
