What Causes Small Genito-anal Ulcerous Lesions

Question: I am currently on vacation and had intercourse with a new partner. The sex was very vigerous and i had a lot of discomfort after. The next day my vaginal opening was quite swollen and very tender as well as my anus. I then looked and noticed open sores on both my vaginal opening and anus which are extremely painful. I know i already had an anal fissure which was visible when i first investigated the pain. I have become worried that these are herpes as i can see distinct infected sores in both areas as well as i had an upper respiratory infection in which i read can happen in accordance with a first outbreak. This morning i have also woken with swollen lymph nodes in my groin which i know is also a symptom. I am currently taking azithromycin for the infection. My urine also has a weird smell and is very painful when it comes in contact with the sores. The pain was immediate afterwards and i did not notice any blisters before the open sores. I have attached pictures of all areas. Is this herpes and if so, how do i treat the sores or could it be that it is an infected wound that has spread?

I understand your query and I will try to help you.

As per your reports these small genito-anal ulcerous lesions may be caused from 1.an infection (HSV)
2.or from rough intercourse with a new partner.

But from your symptoms , localization ,time of appearing after the intercourse it seems to be more small infectious ulcerous genital lesions from rough intercourse.

Hence you have done very well that you have took A/B (UNTIL YOU HAVE REACTIVE LYMPH NODES )and have Sitz baths 3 to 4 times per day until they will heal .
You can use and over the counter creams containing lidocaine and A/B.

Thank you. What are some other things i should be looking for if it were to be the hsv virus? Would the infection of the ulcers be the cause of swollen lymph nodes? How long after intercourse do herpes usually occur? I also have one other small bump which i believed to be an ingrown hair. It is not painful at all but a tiny bit itchy. I have attached a photo. Also could you also explain what a sitz bath is?

We can't exclude HSV

Detailed Answer:
Hi dear again.

1.To be a viral infection like HSV ,is an incubation time or a febrile flu-like prodrome (5-7 days) after intercourse.
We can't exclude HSV, only if you have had intercourse with other partner one week before.
Diagnosis for HSV can be made only by taking a swab from ulcerous lesions (viral culture ).

2.Yes,reactive lymph nodes can be from infection of ulcers .

3.Other small bumps(from attached photo) that you have ,haven't relation with HSV, they are ingrown hair.

4. Sitz baths are genital baths with warm water ,and you can add one teaspoon of salt in one pint of warm water.
Than you topical lidocaine 5% gel or ointment is suitable analgesia.
