Question: Hello,

I have been suffering from allergic pharangitis for two yrs now. I have shown my condition to three ENTs... The first one said I have nodules in my vocal chords and the other two said I have a gap between my vocal chords. I am a 34 yr old woman without any children and went through a divorce two yrs ago. My throat hurts all the time and my voice changes and throat starts to hurt when I speak continuously in meetings. This is severely affecting my confidence and I really worry if I will get cured. Do you think I can completely heal my throat without a surgery? Thanks a lot

Thank you for your query.

1. A Stroboscopy will give an accurate diagnosis.

2. Vocal nodules usually do not cause pain, which can be attributed to the allergic pharyngitis and vocal strain. The phonatory gap can be due to the vocal nodules. There is a normal anterior phonatory gap in the female larynx.

3. Early vocal nodules are common and respond to voice rest, speech therapy, anti-allergic medication (such as cetirizine, fexofenadine with montelukast) and a short course of steroids (such as prednisolone 1mg/kg/day, tapering over 10 days, if not contraindicated). An anti-reflux medication (such as rabeprazole with levosulpiride) can be started. Surgery is not required for early vocal nodules if you strictly follow the medical regimen.

4. Allergy can be tested to reduce the burden of allergic pharyngitis. A post nasal drip of mucus should be ruled out.

5. Upload the Stroboscopy images or video for discussing further treatment options.
