What Causes Spitting Of Bright Blood?

Question: Hi
In the past 1.5 I had mild stomach problems. I had extensive blood work, urine test, chest X-ray, abdnominal ultrasound, endoscopy and gastroscopy (and biopsies) and colonoscopy. I was diagnosed with GERD. A few week later I noticed that when I was sometimes spitting, I was spitting saliva and blood. Sometimes it was only slighlty pink and some others more obvious. I visited an ENT doctor how check my throat again. I had nassal endoscopy on the one side. On the other, he said that my diaphragm is anatomically blocked and this was causing me probably chonic sinusitis (he could not insert the camera as it was too narrow).
Should I seek an another opinion? Is there any possibility to be something more serious that has been developed in the meantime? From all my exams the only problem was that on my first blood work my platelets were 135. On the next day, the count them again and there were 156 (on the normal range, they said there is something with the machine and their size...). Otherwise I am 33, healthy and athletic. I do not smoke

Thank you for your query.

1. Get a Sinus CT and an HRCT Chest. Chronic sinusitis and lung bleeds can be ruled out.

2. I would like to see images of the throat on endoscopy. Lymphoid tissue hypertrophy leads to delicate tissue in the throat that bleeds like this. It is common and easily treated.

3. Bright red blood signifies that the source of the bleed is above the stomach. Gastric secretions discolor blood.

4. A PPI ( Proton pump inhibitor such as rabeprazole) and a pro kinetic (such as levosulpiride) should be started.

5. This is unlikely to be anything serious at your age.

1. I had done in the past years Sinus X-ray. I have chronic sinusitis. Can this be causing the problem?

2. Unfortunatelly I do not have them here

4. I took a PPI and my stomach problem is much better

Thank you for writing back.

1. Get the CT Scans done. They will add a lot of information as compared to X-rays. Chronic sinusitis causes a post nasal drip and granular pharyngitis which may be the source of the blood.

2. It is worthwhile getting a Videolaryngoscopy (VL'Scopy) done again.

3. Add a pro-kinetic. It will help control your reflux symptoms even more.
