Noted your history and understood the concerns.
The scar tissue goes through maturity gradually and takes about 9 months for maturation in normal circumstances.
Since the response of the body for the scar tissue at different areas is different, the one at the peri-anal area is typical - it has depth as it is out of a fistulous tract and goes through more wear and tear phenomenon.
Hence it may still be painful a bit on and off.
Hardness is a stage through which the every scar tissue has to go.
The stabbing pain after passing stool remaining for 2-3 days is usually due to local nerve entrapment in the scar tissue.
Hence this looks to be a stage of normal healing and it may take about 9 months since surgery.

I think your are taking the proper medications and should help you.
Wherever possible you can sit on doughnut pillow so that the pressure on the scar bearing area is reduced.
Identify all the foods and beverages that cause constipation and stop them altogether.
Take mild laxatives changing from one to another to avid constipation and hard stools.
