Question: Issue: Sudden and frequent urination

Case:
     1. Stated 7-8 yrs back. During this period shown 3-4 doctors and they advised some medications.
     2. One of the medication taken was Tamdura. This was taken for 3-4 years. Not much improvement was observed.
     3. In 2104 doctor advised Silofast 8 and Daratenod 7.5 mg. This was taken for 2 months. But, no much improvement was observed.
     4. Then consulted doctor in March'15 and doctor advised Dutas T cap (Dutasteride + Tamsulosin Hydrochlorode) and Roliten OD 4MG cap. This was taken 45 days.
     5. There is no much improvements, so doctor advised various scans and test which are enclosed.
     6. Now doctor has advised the following tests:
          Urodynamics
          Micturation Cystourethrogram
     7. Kindly advise, if these test are required or do you recommend some other options.
     8. All reports from 2010 are enclosed. Please advise and let me know if you would like to speak to the patient.

Thanks for your query,based on the facts and report that you have posted it appears that you have voiding and irritative bladder symptoms due to enlarged prostate.

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you have increased frequency of urination ,nocturnal frequency for
urination ?poor stream etc?
2) Are you diabetic?
3) Do you have urgency ?

The medicines that you have been prescribed so far belong to alpha blockers and
5 alpha reductase inhibitors that help to reduce the size of enlarged prostate and smooth muscle relaxants that help to improve urine flow.

It is known fact that efficacy of these drugs wanes away after they are being used for long time.

The urgency that you have is due to over active bladder which is commonly associated with Enlarged prostate and this can be confirmed by doing Urodynaemic evaluation as suggested by your urologist.

I do not find any rationale behind doing Cystourethrogram as it is commonly advised if one is suspected to have stricture Urethra.

You can get your Uroflowmetry study done to know Urine Flow rate This will help to assess to consider the surgical option (TURP)as modality of treatment (as there is no satisfactory response to medical line of treatment.)


Discuss with your urologist about the surgical option (TURP) to resolve the issue for forever.

This is gold standard procedure practiced world wide for treatment of Enlarged Prostate and gives promising results and helps to improve quality of life of the patient.

Thanks for your response doctor. Much appreciated. Following are response to our queries. Kindly advice next steps.

1) Do you have increased frequency of urination ,nocturnal frequency for
urination ?poor stream etc?
Yes. There is increased frequency of urination. Mostly in afternoon. At night it's only 2-3 times. Three time Uroflowmetry was conducted but failed. Because patient was only able to hold around 150ml or urine in bladder.
2) Are you diabetic? Yes.
3) Do you have urgency ?
Urination urgency is occasional. Morning is fine. Afternoon after lunch is kind of urination every hour.
