Question: What would cause leaning to left and back when I got up in the middle of the night with extreme unsteadiness and vomiting?

Your symptoms could be related to an inner ear disorder or low blood pressure.
Have you measured your blood pressure?
If your symptoms persist I recommend going to the ER for a physical exam and some blood lab tests (complete blood count, blood electrolytes, kidney and liver function tests, fasting glucose).
An ENT specialist consult may be needed, to rule in/ out an inner ear disorder.
A cervical spine x-ray study is also necessary to exclude chronic backbone degeration as a possible cause of dizziness and nausea.
