What Causes Sudden Explosion In The Middle Of Forehead While Having A Bowel Movement?

Question: Greetings: my 31-yr-old daughter, while having a bowel movement, had a sudden "explosion" in the middle of her forehead, followed by extreme dizziness, sweating, and numbness to the extremeties. This is the 2nd time this has happened, while having a BM, and the first time she actually passed out. This is scarey!! Please advise.

I am Dr.Ajay Panwar,a neurologist,here to answer your query.

If your daughter would have presented to me as an outpatient with this complaint,I would have urgently evaluated her for signs of meningeal irritation and for any neurological deficit,considering the possibility of 'Thunderclap headache' as my working diagnosis.
Common causes of Thunderclap headache are-
1)RVCS(Reversible vasoconstriction syndrome)
2)CVT(Cortical venous thrombosis)
3)Subarachnoid hemorrhage
4)Brain aneurysm
5)Drugs like cocaine and sympathomimetics
6)Accelerated hypertension

First step would have been to get an urgent plain contrast tomographic scan(plain CT scan) of Head to see for any vascular event as listed above(1-4).
A CT angiogram may also be needed.Ophthalmologist opinion for funduscopic examination is also needed.

Whatever,she needs urgent evaluation.Please rush to Emergency room.
