Brief Answer:
Scrotal swelling

Detailed Answer:
Hi,

Thank you for your query. I can understand your concerns.
Scrotal swelling appearing while urinating and disappearing soon thereafter indicates indirect inguinal hernia which is increasing and getting pushed into the scrotal sac while intra-abdominal pressure is increased with Valsalva at passing urine.

Alternatively vaginal hydrocele which has communication with /intr-abdomanl extension is a possibility. You have to undergo physical examination and ultrasonography of scrotum and its contents.
