Dear sir, I have gone through your question and I understand your concerns . it seems that you are suffering from balanitis which is inflammation of the foreskin leading to its swelling and possibily itching. This can be caused by a number of reasons including skin irritation with scented soap or uncontrolled diabetes that predispose to infection. Another causes include fungal or bacterial infection.. if you have a discharge as well then the cause might be a sexually transmitted disease as syphilis , gonorrhea or trichomonus.
if you have discharge then you probably will need a swab test and a urine analysis to diagnose the causitave organism.
if you do not have a discharge then your balanitis is probably caused by a fungus (a type of yeast infection) In this case, you can use an antifungal cream that contains nystatin, clotrimazole, or terbinafine.
Topical steroids that contain hydrocortisone can also be helpful to reduce swelling and inflammation together with a topical antibiotic cream to eradicate any possible bacterial infection as well.
