Question: Hello Doctor,
After lot of study over the internet on the problem I have, I have understood that I have external Hemorrhoid. A small lump outside the anus. It swells during the passing of stool and when its hard to pass stool, this swelling bleeds. After that it more or less disappears with a piece of skin in the place of swelling. Could you pls. suggest me the best possible treatment or medication ? looks like 1st degree hemorrhoid. Bleeding is not always and its only when I have hard stool or constipation.

Brief Answer:
its not hemorrhoids

Detailed Answer:
Hi XXXXXXX

Sorry to disappoint you, but as per your history and symptoms, it doesn't seem to be hemorrhoids, neither external nor internal. It seems most likely to be 'chronic fissure in ano'.

Hemorrhoids are usually painless and not associated with passing hard stool. It's fissure that is associated with these.

Every hard stools stretches the anal canal leading to tear. This causes pain and bleeding. And due to this pain and tear, the anal sphincter goes into spasm leading to constipation and the cycle continues. And when these tear heal, they form a skin tag protuding outside, which you got confused with external hemorrhoids.

Laxatives to prevent constipation will be the first line of treatment. Sitz bath for 10 - 15 minutes can be effective. You can also apply a local anaesthetic gel at the anal opening before and after defecation to ease the pain.

Hope I have been helpful.
