What Causes Swelling Of Foreskin On Penis Tip?

I can understand your concern for mid swelling with tenderness over the foreskin and continuous discharge from the same.
After analyzing the query with best diligence and also keeping the episode of condom break, it appears to be a normal physiological phenomenon.
To tell you elaborately:
In most of the sexually transmitted infections with discharge and redness, these occur within 7-14 days of intercourse and that too unprotected intercourse. It is very rare to find any Sexually transmitted disease after condom break and even if it is present then manifests itself within 1-2 weeks.
In your case I can make out that the condom break episode occurred almost 1-2 months ago. This makes you almost clear from the part of Sexually transmitted diseases related to discharge and redness.
I can also make out that you are uncircumcised male, so it is quite common in uncircumcised males to get such kind of trivial episodes. Most of the times this is due improper hygiene and it can be managed with proper care.
I suggest you following measures to take care of your problem:
-Wash your penile area for two times in a day. You can use a mild soap to wash the area. Wash it very gently but properly. If you are uncircumcised then it is quite important to retract your skin fully backwards and then wash the area.
- After every sexual intercourse too, wash the area properly. I personally suggest you to use protection in form of condoms during every intercourse. –
- Wear cotton undergarments. Change these two times a day. Do not wear any synthetic or tight garments.
I suggest you to drink plenty of water to flush your urinary system which can also cause mild symptoms as you are having.
Do keep a watch over the area for next 7 days. If this condition persists, then I suggest you meet a Sexually transmitted disease specialist or an urologist.

So far I've been washing it carefully and the redness has gone down. It continues to itch and the foreskins hole remains small.
Also the semen like substance seems to be coming from underneath the foreskin. It is a yellowish color. Also, I attempted to masturbate to see if everything was working, and was surprised to see blood coming from underneath the foreskin. To clarify, it is not coming out from the penis, it comes from the space between the penis and foreskin.
Do not masturbate and fiddle with the penile skin.

The penile skin at this time is quite fragile and therefore I can get weakened further by Masturbation or by fiddling. The opposing surface between glans Penis and frenulum can get peeled off by the masturbation or repeated touching.
I suggest you to not even unnecessarily touch the area. I suggest you to continue to wash the area very gently as guided before.
