Question: I have been using combination of Amlodipine 10mg and Atenolol 50 to control my blood pressure for last 10 years. Recently i am having foot swelling and one night felt breathing trouble. Is it side effect of this medicine. Should I change the medicine and if yes, what will be proper one? Recently I am having cholostorel (254)

Brief Answer:
No need to change medicines

Detailed Answer:
Hi and thanks for your query.

I am not sure at which area of foot you are experiencing swelling. Amlodipine is associated with ankle edema. Even if ankle edema (swelling) is present, it is not an indication to change this medicine.

Atenolol can cause breathing problems especially if one has history of asthma or chronic obstructive lung disease. If you do not have any such history, then an occasional mild episode of breathing difficulty can be ignored.

Well there is some chance that foot swelling and breathing difficulty is not due to medicine. In such a case an ECG and chest X-ray may be done. Seeing an internal medicine specialist would be needed.

Hope it helps.

Thank you for the reply. The swelling is with ankle portion. I have seen a doctor. Regarding breathing problem he has not done ECG but told after outward checking that there is no sign of a cardiac problem. However, I will get ECG and chest x-ray done, if you suggest.

Should I take medicine for cholostoral, which doctor is telling is on higher side (bad cholostoral reading is 254 instead of normal count of 180 max). He has prescribed for Atorvastatin 10mg for 3 months. I have some kind of throat allergy also and taking montelukcast+levocetrizine occasionally.

Brief Answer:
Need your reports

Detailed Answer:
Hi again

I need your reports of lipid profile, ECG and chest x-ray. Then only I will be better able to help.

Throat allergy should not cause breathing problem with atenolol. If lung is congested then it can cause the problems.

It is good that examination does not suggest cardiac problem.

Kindly upload the report of lipid profile.

Brief Answer:
You dont need medicines

Detailed Answer:
Hi again

Thanks for uploading the reports.

Your LDL levels are 177. The recommendations are to try lipid lowering medicines when LDL is more than 190. So according to recommendations you still need to try low fat diet and exercise.

There are some reports that lipid lowering agents help lower blood pressure as well. Atorvastatin is a medicine that can occasionally affect liver and muscles. Your liver is normal. Thus you are suitable candidate for lipid lowering agents.

Mostly if you can reduce your cholesterol with diet and exercise then you do not need lipid lowering agents. Otherwise a short trial can be taken but long term trial is still not recommended. Short term trial can be from 1-3 months.

Hope I made it clear.

Feel free to ask more doubts.
